Admission Process

1) Please email info@cerecare.org.cn or call 86-21-54394328 to make an appointment with the rehab department for a free consultation and preliminary assessment.

2) Please bring your child to the appointment so that our rehab department and medical department staffs can make a primary assessment and examination. Get a copy of the “Admission Information and Preparation Checklist” (hereafter called the “list”).

3) Go to the hospital for a physical examination and prepare the materials from the list.

4) Bring your child as well as the requested materials from the list to CereCare for the medical department to sign and validate them.

5) Please fill out the Registration Form for our records so we can determine a personalized rehabilitation plan for your child.

6) Please pay and finalize the admission process at the finance department, then officially register your child with the guard at the entrance.
