Medical Records Checklist

After your child passes the medical examination at the hospital, please bring the following materials upon enrollment.

1. Medical insurance card, medical records, and diagnosis report. Please provide as much information as possible.

2. Routine urine test (RL), routine stool test reports

3. Routine blood test, liver function test (GPT) reports

4. Electrocardiogram (EKG/ECG), encephalogram (EEG), chest X-ray reports

5. Vaccination records
